What is an Episiotomy?
An episiotomy is an incision made into the area around the vaginal opening (perineum) either towards the anus or at an oblique angle, to enlarge the space through which the baby must pass. This is generally done to expedite delivery or to prevent spontaneous tearing of the perineum - as a controlled cut is thought to heal better than a random tear.
At one time, episiotomies were almost standard procedure in hospital births. However, they have become somewhat controversial due to the possibility of complications and research that has shown that episiotomies may actually increase pain and recovery time after childbirth. A well prepared mother and a skilled midwife can greatly reduce the need for an episiotomy which should only be performed when necessary and for the benefit of the mother and baby, rather than the convenience of the delivery team! Many professional midwives pride themselves in being able to deliver babies without the need for episiotomies or risk of perineal tear, by skilful guiding and coaching of the mother during the second stage of labor.
The Natural Way
It is very important to outline your birth preferences in a birth plan – are you keen to have an episiotomy performed or not? Discuss carefully with your obstetrician or midwife how often they perform episiotomies, under what circumstances this procedure is done and whether tearing can be avoided. To help prevent an episiotomy from occurring, perineal massage may be done at least five to six weeks before the delivery using a natural, therapeutic grade, non-scented massage oil. How do I use Easiotomy Cream?
Easiotomy Cream can be used both before and after birth to prevent and heal perineal damage.
DIRECTIONS:
For a few weeks before birth, the cream should be lightly massaged into the affected area one to three times a day, after cleansing. After birth, gently apply the cream once or twice a day after cleansing.
How long until I see results?

Q: How long does it take for an episiotomy to heal?
A:
This can vary from individual to individual. Depending on the depth and extent of the incision you can expect to feel uncomfortable for between 2 and 6 weeks. If you have not had an episiotomy and have not torn during delivery, but have general bruising or perineal abrasions, the healing time will be much shorter.
Q: I have heard that my first bowel movement after birth will be difficult. Is this true?
A:
Many women are worried about this when the time comes. The best advice is to try not to worry and to allow nature to take its course. One often does not have a bowel movement for a day or two after giving birth and this usually gives you a chance to recover. Make sure that you eat lots of fresh fruit and vegetables and especially that you drink plenty of water to prevent constipation and encourage a soft stool. Once you have your first bowel movement, you will probably realise that your anxiety was greater than the actual discomfort experienced!
Q: How long before we can have sex again?
A:
This is a very individual matter and you should only resume penetrative sex once you feel ready. Generally if you have had stitches, it is wise to wait at least until the stitches have healed, which can take 4 - 6 weeks. If you managed to give birth without an episiotomy, you may feel ready sooner. The best idea is to take advice from your health care provider during your postnatal check ups and to follow your own feelings.
Initial tenderness and some anxiety is to be expected when making love for the first time after childbirth. However, relaxation, a patient and understanding partner and some lubricant if necessary will go a long way to making you more comfortable.

What else can help?
Cold packs or compresses placed on the perineum can help to relieve the worst of the pain in the first few days after delivery. Special compresses for the perineum may be provided by your hospital or birthing centre or may be bought from medical supplies stores. However, even a clean cloth soaked in a little lavender or rose water and applied to the perineal area for a few minutes will provide welcome relief.
'Sitz baths' or sitting in a clean tub filled with cold or warm water will provide relief from pain and reduce swelling. Adding a handful of salt to the water will also help to prevent infection.
Use a squirt bottle to decrease burning pain when urinating. Squirt your perineal area with water while urinating as well as afterwards for a gentler option than wiping.
